Let's carry this free needle thing just a bit further. Let's think outside the box.
What is really needed is a direct injection pump, like those insulin pumps that some diabetics use. A measured dose is delivered automatically so that the user never need to come down. Add a feeding tube and waste plumbing and stack the addicts in bunks in warehouses until they die. Costs could be controlled through automation and from direct contributions BY PEOPLE WHO CARE.
Grade & high schools could have field trips to these warehouses to show consequences of opioid use. Crime would drop. No more drug addicted children. No more spreading of diseases. Drug dealers would go out of business. What's not to like.
